What is perineoplasty?

Perineoplasty is a surgical procedure that is performed to repair the muscles and the connective tissues that have been damaged due to extreme stretching and tearing in the perineum. The perineum is the area that lies between the posterior end of the vaginal opening and the anus.


Perineoplasty is performed when the space between the rectum and vagina has shortened due to the damage done to the tissue.

Perineoplasty is specifically the tightening of the muscles and removing the excess tissue and skin from the perineum. Perineoplasty is performed to reduce the vaginal opening as well.


Most women also face loose vaginal muscles along with damaged perineum, both vaginoplasty, tightening of the muscles along the vaginal canal, and perineoplasty are performed together for them.

Why do women need to go for perineoplasty?

There are a few reasons for performing this treatment. One of the main reasons is the wider opening of the vagina along with loose muscles on the posterior opening of the vagina. This can also be combined with loose muscles between the vagina and the rectum area.   

Wider Opening of Vagina

The main reason for having this loosening of muscle tissue and the wider opening is going through one or multiple vaginal deliveries. These deliveries have been very destructive to the perineum tissues. Usually, women are not stitched up as they are supposed to after the delivery and therefore, with passing time, the torn muscles loosen up.  

Aging and Obesity

The perineum muscles can also loosen up with age and due to obesity. Even though these are rare cases, but they also are the reasons for having loose muscles down there.  

Sexual Satisfaction

Women who want a smoother appearance around the vulva, which they had before the perineal tissue was damaged, would go for perineoplasty. They would want a smaller vaginal opening to experience the sexual intimacy as they had before with their partner. The base of the vaginal opening creates sensation for both male and female partners, and loose muscles in that area can cause reduced sexual intimacy during intercourse.

Another reason that is found common in women is a recurring infection in the vagina as well as increased frequency of UTI. This is because the reduced space between the anus and the vagina can cause harmful bacteria to travel towards the vagina and urethra.

How is perineoplasty performed? How long does it take?

It is a day’s procedure. Patients do not need to stay overnight, just a few hours. It is done under local anesthesia and it takes around 45 minutes to an hour to perform this procedure.

First of all, a V-shape marking is made on the skin that is falls right below the vaginal cavity. The top incision marking can be flat, curved or angled upwards. This varies depending on the degree of tightening that is needed.

Once the marking is done, local anesthesia is injected in that area. It also has some amount of adrenaline, which helps minimize the amount of bleeding.

A while after the area has been numbed, the incision is made on the marking to cut the skin and expose the tissue underneath. The muscles are then brought together, tightened up and realigned with stitches. This improves the condition of the pelvic floor muscles and restores some tightness to the vagina.

Once the muscles inside are stitched together, the skin is also stitched up with dissolvable sutures.

The patient can go home once the procedure is done but needs to take extra care for the next few days. As the numbness wears off around 3 to 4 hours after the procedure, the patient might feel and be swollen along with a little degree of pain in and around the area of surgery.  


What is the recovery time?

It takes about 5 to 7 days for women to recover from this surgery. They can get back to their normal routine in the next couple of days post-surgery, but they need to also take precautions until the sutures dissolve and fall off, which is for the next 4 to 6 weeks.  


What are the post-op precautions?

Avoiding any sort of penetration into the vagina – which includes having sexual intimacy, tampons or menstrual cups etc.


  • Try to maintain a healthy diet to avoid constipation
  • Refrain from any strenuous activity like bending too much and lifting heavy objects
  • Take the medication prescribed to you on time

How does perineoplasty affect my sex life?

The procedure is performed to tighten the loose muscle on the perineum and close the opening of the vagina.  Usually, this condition is accompanied by loose vaginal muscles as well and therefore, perineoplasty and vaginoplasty are usually performed together.

The base of the vaginal canal, where the tissue is mostly damaged, is the one that is tightened. This is the tissue that arouses both the male and female
during sexual intercourse. The tighter vagina can grip the penis, creating the sensation that occurs as the skins of both genitalia are rubbed against each other.
